Transition Services
Riverside Family Support Center offers transition services to children and adolescents with disabilities living with their families who will soon be assuming adult roles in their community. Transition services are structured to help individuals and their families navigate the many steps to ensure that the individual is set up with the right services and state agencies as an adult, so that they can continue to work on achieving their goals. These goals can include employment, post-secondary education, maintaining a home, becoming involved in the community and building relationships. This process should be started as soon as possible, to make sure all necessary supports are in place by the time the individual is 22 years old. See a timeline of steps here.

Family Support Staff is available to discuss any questions and concerns and to provide the following:
- Help creating a plan for your child’s future
- Information on upcoming workshops and trainings on Transition
- Age-appropriate information packets discussing various supports available
- References to the appropriate agencies
